Park Nicollet – SmartCare Clinis is seeking a 0.6 FTE Family Medicine Nurse Practitioners or Physician Assistants to join their growing, collaborative care team.  Our SmartCare Clinicians Deliver comprehensive services in health promotion, screening, diagnosis and disease management through alternative delivery methods such as e-visits, telephone visits, and video visits with fewer in-person traditional office visits. Provide team based care with other highly skilled physicians, advanced practice clinicians and nurses who will practice to the fullest extent of their training and licensure.

Successful Candidate Musts:

• Desire to provide the best care experience for our patients
• Ability to critically and creatively evaluate alternate ways of delivering care
• Enjoy and thrive on making sense out of ambiguity 
• Readiness to partner effectively with others – panels will be shared across the entire team
• Willingness to have a fluid and flexible schedule for our patients, including same day care, extended hours, rotating Saturdays 8am – 12pm, and virtual care days.
• Proficient in technical speak and patient friendly terms and language
• Minimum of 1+ years of outpatient primary care clinician experience

Our specialty center is frequented by many of our patients and is conveniently located in the thriving area of St. Louis Park. A diverse community, St. Louis Park features walkable neighborhoods and beautiful, family-friendly outdoor spaces. There are over 50 parks as well as miles of biking and walking that connect St. Louis Park to downtown Minneapolis and neighboring communities. Our specialty center is also just a five-minute drive to Uptown, Minneapolis, where locals enjoy a popular chain of lakes for walking, kayaking and relaxing with family and friends as well as many wonderful dining options. We’re also a 10-minute drive from downtown Minneapolis and a 20-minute drive to the airport.

PAY RANGE STATEMENT 
Compensation is based on the level and requirements of the role. Pay within our ranges may also be determined by education, experience, knowledge, skills, location, and abilities as well as internal equity. After the salary guarantee period, this position will be compensated based on work RVU productivity.
